A Novel Laser System Using SPACE for Therapeutic Endoscopy

In the era of therapeutic endoscopy, a new device for precise cutting and simultaneous hemostasis is required. Laser technology allows immediate vaporization of tissue. The thulium laser is a new type of surgical laser. It emits a continuous 2-μm wavelength delivered through a thin, flexible laser fiber. Its penetration depth is approximately 0.25 mm, which is suitable for tissue resection without deep injury. Theoretically, continuous-wave mode of a thulium laser facilitates precise cutting and effective hemostasis. Air insufflation is required during endoscopic procedure to achieve adequate visualization of the gut lumen. Flexible gastrointestinal endoscopy is performed under on-demand insufflation. However, gut lumen can be excessively distended by pressing the air button without pressure monitoring. Recently, a new overtube system has been developed for steady pressure automatically controlled endoscopy (SPACE) in the gastrointestinal tract. During gastrointestinal endoscopy, SPACE might prevent the patient`s discomfort and procedure-related complication caused by excessive insufflation.
